GoDog Transportation Authorization & Liability Acknowledgment

Transportation Authorization I authorize GoDog to transport my dog(s) to and from veterinary, grooming, daycare, boarding, training, or other approved destinations.

Health Acknowledgment I certify that my dog is in good health to the best of my knowledge and free from any known contagious illness. GoDog maintains clean vehicles and routinely sanitizes equipment, restraints, and transport areas. However, dogs may be exposed to viruses, bacteria, parasites, and other environmental risks while visiting veterinary clinics, grooming facilities, boarding facilities, public spaces, and other locations beyond GoDog's control. By using GoDog's services, I acknowledge and accept these inherent risks.

Emergency Care If my dog requires emergency veterinary care while in GoDog's care and I cannot be reached, I authorize GoDog to seek treatment on my behalf. I understand that I am responsible for all related expenses.

Behavioral & Medical Disclosure Please note any medical conditions, medications, anxiety, aggression, escape tendencies, or special handling needs in the reservation form.

Liability Acknowledgment GoDog is committed to providing safe, attentive, and compassionate transportation for every dog. While every effort is made to ensure a safe and comfortable experience, transportation involves risks that cannot be entirely eliminated. I acknowledge these risks and agree that GoDog shall not be responsible for illness, injury, loss, or damages resulting from circumstances beyond its control.

Client Certification By submitting this reservation, I confirm that I have read and understand this agreement and voluntarily authorize GoDog to transport my dog.
